Prescriptions and Renewals
New prescriptions and narcotic refills require an office visit. Requests for routine refills must be made during normal business hours. They will not be called in by the on-call physician after hours. Please make every attempt to call 2-3 days before your prescription runs out in order to assure no lapse in treatment.

Financial Policies
Payment is expected at time of service. This includes co-payments, deductibles, fee for service, and services not covered by your insurance plan. We accept cash, check and American Express / Visa / MasterCard. There is a charge for non-sufficient fund checks, missed appointments, and medical record copies.

Aside from Medicare or an insurance plan contracted with Fox Valley Internal Medicine, the patient is responsible for submitting secondary insurance claims. Our billing department will be happy to provide you with the appropriate insurance form. If there are payment concerns, please notify our billing department and they will be glad to assist you.

Managed Care and Referrals
Referrals for specialists, procedures or tests may take 5-20 business days to be completed. Proceeding before approval may result in the patient being responsible for the bill. Prior to any appointment, confirm that the specialist is still in your insurance network. It is your responsibility to assure that specialist referrals, tests and procedures are covered under your plan. We make every attempt to refer to conveniently located physicians.

Occasionally a request for a needed medical service may not be covered under your insurance plan. The receptionist staff and bookkeeper will try to determine if the service is covered or not. If not covered by your plan, you will be required to pay at our regular professional fees. Depending on insurance company response time, you may be informed of non-coverage after the service has been performed.
